• Provide Borang 9, 24 & 49 SSM (Mydata-Digital Certified True Copy)

If you are applying a Main to look after child caretaker, the document to be submitted are as follows:

• Child must be 15 Years Old Below

• Dependents of I/C

• Birth of Certificate 

 

If you are applying a Main to look after a sick parent or parent-in-law, the documents to be submitted are as follows:

• Photo of the patient / old folks Sitting on Wheelchair

• Elderly of I/C

• Doctor's Medical Letter (Original) by Government or Specialist Hospital. 

• Birth of Certificate (as proof of the relationship between employer and sick parent/parent-in-law

If you are applying to look after a big house, semi D and bungalow, the documents to be submitted are as follows:

• Maximum of 6 person in household

• Family member of I/C

• House number

• road sign

• Photo of front, backside, inside living room, bedroom, bathroom, kitchen, garden and others
